This document is a Parade or Event Permit Application for the Borough of Ebensburg, Pennsylvania. It outlines the requirements and procedures for organizing a parade or event within the borough’s jurisdiction.

Application Form Details

The application form requests:

• Applicant Information: Name, address, phone, email, fax.

• Public Land Use: If public land or streets will be occupied, specify which ones.
• Services Needed: Electric service and police assistance (circle YES/NO).

Additional Requirements

Private Property Use

If private property is used for parade formation or disbanding:

• Provide the name, address, and phone number of the private property owner.
• Attach a letter of permission from the private property owner.

Certificate of Insurance

A Certificate of Insurance is required, covering:

• Public Liability: Not less than $1,000,000 for injuries, including accidental death.
• Property Damage: Not less than $1,000,000. The insurance must name the Borough of Ebensburg as an additional insured and be provided by a company authorized to do business in Pennsylvania.

Important Notes

• The Borough of Ebensburg reserves the right to withdraw permission if any requirements are not met.
• The Ebensburg Police Department may also withdraw permission due to severe adverse weather or traffic conditions that make the event unsafe.
