Application for Water/Wastewater Service – Ebensburg, PA

This is an official application form from the Borough of Ebensburg for residents or businesses requesting new water and/or wastewater service. The form must be completed in full, with supporting documentation attached if applicable.

Key Information Required:

  • Applicant Details: Name, service address, mailing address (if different), city/state/zip, phone number, fax, email.
  • Ownership Status: Indicate whether you own or rent the property.
  • Engineer/Contractor: Name of professional involved in installation (if any).
  • Landlord Info (if renting): Full name, address, phone, fax.

Important Notes & Requirements:

  • The Ebensburg Water & Wastewater systems are owned by the Ebensburg Municipal Authority and operated by the Borough.
  • Applicants must agree to follow all current Rules & Regulations, rates, and fees — available at www. ebensburgpa. com.
  • All tapping fees and obligations must be paid in full before connection.
  • The Authority may conduct usage analysis; overuse beyond original allocation can result in additional fees (per Section 5. 3. 6).
  • Attach a development or lot plan showing locations and sizes of all water/wastewater lines, fittings, and appurtenances.

  • Estimated Usage in EDUs:
    (1 EDU = 165 gallons per day for water / 200 gpd for sewer)
